入职核查清单（员工使用）

Congratulations on receiving an offer to join us. We welcome you to join our family!

祝贺您收到了我公司的聘用函！欢迎您加入我们的大家庭。

It is important for you to understand our on-boarding procedure. To ensure a smooth on-board process, please carefully read the information and provide required documents and materials on your first day of joining:

了解我们的入职流程对于您来说是非常重要的。为了确保您的入职流程顺利进行，请您仔细阅读下列信息，并在入职日当天准备好相关文件及材料：

I. Work hours and Dress code

工作时间和穿着要求

1．Office hours

Standard working hour:

Office 08:00~17:30

Dining break : 1.5 hour

用餐时间：1.5小时

2．Appearance

仪容仪表

Your clothing should be professional, suitable, openhanded and neat.

衣着要职业、得体、大方、整洁；

Shorts,slippers, sun-top, transparent clothes are not allowed.

不允许穿短裤、拖鞋、吊带上衣或透明服装上班；

Ladies cosmetic and ornament should be properly .

女士化妆及修饰应该得当；

Neat haircut with suitable ornament.

发型整洁，修饰得当。

II. On Boarding Procedure

报到办法

Make sure that you have given us a reply according to the offer letter requirement.

确保您已按聘用函的要求，给予我公司回复，表示您接受我公司的录用。

Attend the new employee orientation; understand the company, business, rules and regulations.

接受我公司的新员工入职培训，了解公司介绍，业务介绍，规章制度等。

III. Document

报到所需文件

ID Card

大陆居民身份证明

ID Card with one copy; Provide your Hukou and certificate proved by police station if your ID Card Ver 1.00

身份证原件及复印件1份，若身份证遗失，须提供户口本原件，及户口所在地公安机关出具的身份证遗失证明。

Education Certificate

最高学历证明

Original and photocopy of Graduation certificate, degree certificate in additional if bachelor degree. 学历毕业证书原件及复印件1份。如您具有本科及以上学历，请同时携带学位证书原件及复印件1份。

Employment separation certificate

劳动关系终止证明

Provide your employment separation certificate with previous company, if experienced.

非首次工作的人员，需要提供与前一家工作单位劳动关系终止的证明材料，并由当地相关部门盖章。如退工单，劳动手册，就业登记证。

Employment agreement

就业三方协议

Provide your employment agreement, if graduates.

如果是高校应届生，请提供由学校盖章的就业三方协议，以便办理正式录用。

Bank account

薪水支付账户

Provide your account number and name in bank of china, with one copy of Bank Card, using regular savings account is preferred

提供中国银行的银行账户账号、开户行名称及银行卡复印件1份，建议使用居住地的普通储蓄账户。

IV. Medical Examination

体检

We accept the general medical examination report in three months from the day when we send your offer,but the report has to be certified by Disease Control Center which has national secondary qualification or above.The report should contain the items as below: 我公司接受自聘用函发出之日起，前三个月内常规项目体检报告。但该报告需由二级资质以上疾病控制中心出具。

特别提醒：

The position we offered may need business trip , it is not suitable for the person who can not fatigue because of physical reasons.We suggest you had better do the examination of two pairs of semi-hepatitis B and make careful decision whether you can accept our offer if you get an abnormal results.

我公司提供岗位可能需要您出差，不适合因身体原因不能疲劳者，建议做乙肝两对半的检查。如检查结果异常，如谷--丙转氨酶指标偏高，乙型肝炎表面抗原呈阳性者，请慎重决定是否接受我公司的岗位。
