轮椅租赁协议

Wheel Chair Rental Agreement

我同意接受下列租赁协议：

1. 酒店仅为住店客人免费提供轮椅租赁服务。

2. 我已对我所租赁的轮椅进行了检查，并愿意自己承担风险。

3. 酒店对我使用轮椅期间所发生的任何意外或事故不付法律和经济上的责任。

4. 如我不小心丢失了我所租赁的轮椅，我将要付给酒店人民币 1000元做为赔偿。

5. 在我使用期间对轮椅造成的任何损坏，我都将要付给酒店相应的修理费加上20%的手续

费。

I agree to the following conditions for renting the Hotel Wheel Chair. for in house guest only.

1. Hotel offers Wheel Chair rental service with free of charge

2. I have personally checked the Wheel Chair and take this trip at my own risk.

3. The Hotel undertake no legal of financial responsibility for any accident or any other incident

during the Wheel Chair use.

4. In case of the loss of Wheel Chair, I will pay the Hotel RMB1000 in compensation.

5. For any damage caused to the Wheel Chair during my rental, I will pay the cost of repair plus

20% service charge to the hotel without any arbitration.
